The Coalition of Community Health and Resource Centres

The Community Diabetes Education Program of Ottawa works closely with partner agencies to facilitate the delivery of diabetes education programs at multiple locations throughout the city of Ottawa.These agencies are all part of the coalition. Canadian Diabetes Association (CDA) 

The Community Diabetes Eduction Program of Ottawa works closely with the CDA to offer evidenced based diabetes care. Such activities include presentations to special groups/ staffing displays and booths at the CDA biannual diabetes Expo, working with community groups to offer risk assessments. Ottawa Public Health

Through resource sharing and cross referrals CDEPO works with the Diabetes Prevention Department of the City of Ottawa Public Health to reach clients from all walks of life. It is this special partnership that brought such great events as the First Nations, Metis and Inuit Diabetes Network/ Diabetes Health Symposium.

Projenesis

This is a volunteer run program offering diabetes risk assessments and screening for the Hispanic community of Ottawa. Through ongoing collaboration with several partners, Projenesis is able to support the efforts to prevent diabetes in a high-risk population.

Champlain Diabetes Regional Coordination Centre (DRCC)

Our mandate
is to promote regional integration of services by bringing clarity to roles and addressing gaps in services and in the hand-offs between services, engage diabetes education programs across the Champlain region to promote service integration & adoption of best practices.
